Help with authorized daily personal-care needs
DMH Personal Care is based on the individual's approved services and support plan. Superb Caregivers coordinates trained personal-care staff and communicates with the individual's authorized support team.
Services are non-medical and are limited to authorized tasks. Availability depends on current authorization, individual needs and staffing.

Confirm authorization
The appropriate DMH authority or support team determines eligibility and authorized services.
Coordinate services
After the required referral and authorization, we coordinate staffing and service setup.
Referral partners: We welcome calls from support coordinators, case managers, community mental-health professionals and discharge planners.
